About MileStem
MileStem™ is being built as a practice operating system for pediatric therapy teams — centered on the child record, shaped by daily documentation realities, and designed for practices that need clarity without enterprise bloat.
MileStem exists because small and independent pediatric therapy practices are often forced to choose between paper-heavy processes, generic tools, or large systems that feel built for someone else.
The goal is simple: give providers, supervisors, and practice leaders a calmer way to manage visits, plans, forms, signatures, parent communication, supervision, and required records from one organized workspace.
Instead of treating documentation as a stack of separate PDFs, MileStem is built around the child’s service record — so the team can see what has happened, what is due, and what still needs attention.
What guides the platform
Documentation should support the visit, not pull attention away from the child and family.
Visits, forms, signatures, goals, documents, and communication should stay connected to the child’s record.
Supervisors and administrators need visibility into pending work, missing items, deadlines, and documentation status without chasing updates.
Completed documentation should be easier to review, share when appropriate, and keep audit-ready.
Practices should be able to start with the workflows they need now and add new service lines as they grow.
Access control, tenant separation, document handling, and audit-conscious activity should be part of the product, not an afterthought.
Where MileStem is focused
State-aware intake, service coordination, IFSP, family training, transition, consent, signatures, and required documentation workflows.
Speech records, evaluations, plans of care, goals, authorizations, session notes, documents, and billing-support workflows.
Supervision-heavy workflows for candidates and supervisors who need organized hours, records, verification support, and audit-ready documentation.
ABA, OT, and PT workflows are being built from the same modular, child-record-centered foundation.
We’re working with a limited number of pilot practices and welcome conversations with providers who want a better way to manage documentation, supervision, signatures, forms, and daily follow-up.
